Download Instructions

To download image to your computer click on the image.  This will then take you to an enlarged view of the picture.  Then choose one of the following options.

1. Once you move your cursor over the image, a pop up menu should appear.  Select the disk icon.  When the menu appears, select area you wish to save to on your computer. 
2. Move your cursor over the image and right click.  A drop down menu will appear and choose save picture as and then select area you wish to save to.
3. Go to the file menu and choose save as and then select area you wish to save to.
4. To return to download section click on back button.

Download Instructions

1. Scroll down the list and find the form you are looking for.
2. Click on the icon to view document (format represented by the icon).
3. Document can now be edited and is ready for use.
4. To save document to your computer, go to the file menu and choose save as and choose file location for saved document.
5. To return to download section click on back button.
